DAISY Awards at CMH

Citizens Memorial Hospital/Citizens Memorial Health Care Foundation is proud to be a DAISY Award partner since 2019. The DAISY Award is an international program that rewards and celebrates the skillful, compassionate care nurses provide every day.

The DAISY Foundation was established by the family of J. Patrick Barnes after he died from complications of the auto-immune disease Idiopathic Thrombocytopenia Purpura (ITP) in 1999. DAISY is an acronym for Diseases Attacking the Immune System. The Barnes Family was awestruck by the clinical skills, caring and compassion of the nurses who cared for Patrick during his hospital stay. The family created this international award to say thank you to nurses everywhere.

At CMH, we are proud to offer the DAISY Award for registered nurses (RN) and licensed practical nurses (LPN). The DAISY Nurse Leader Award at CMH is for nurse managers, directors, assistant managers, charge nurses, educators, chief nursing officers, preceptors, informatics nurses and patient flow nurses.

Daisy Award

Say thank you by sharing your story of how a CMH/CMH Foundation RN or LPN made a difference you will never forget. The DAISY Award committee selects one nurse each month who exemplifies the following characteristics:

  • Models empathy and demonstrates a caring attitude in all situations.
  • Demonstrates extraordinary clinical skills in the delivery of compassionate patient care.
  • Exemplifies the mission, vision and values of CMH/CMH Foundation.
  • Establishes a special connection with patients and families.
  • Is an outstanding role model for the nursing profession.
  • Has significantly affected the life of a patient/family by going above and beyond.

Each month’s winner receives a DAISY Award honoree pin, a Healer’s Touch hand carved statue, an award certificate, copy of the nomination, and DAISY bag. In addition, the selected nurse’s unit/department/clinic/facility receives a banner to post for the month and freshly baked cinnamon rolls for everyone on the day the award is presented.

DAISY Nurse Leader Award

As the healthcare landscape becomes more complex, with increasing demands on nurse leaders, we believe that those who create an environment of compassion and recognition for others strongly deserve to be recognized themselves. If you have a nurse leader you would like to nominate, describe their impact on staff and patient care they manage using the criteria below. The DAISY Nurse Leader Award committee selects three nurse leaders a year who exemplify the following characteristics:

  • Creating an environment where attributes of trust, compassion, mutual respect, continued professional development and ethical behavior are modeled and supported
  • Being accessible, available, and responsive to the needs of their team
  • Problem-solving and encouraging critical thinking
  • Advocating for their teams
  • Ensuring that the needs of the teams are communicated to organization leadership and organizational priorities and directions are communicated to their teams
  • Being compassionate leaders

The selected honoree will receive a special DAISY Nurse Leader certificate in a DAISY Foundation portfolio, a specially designed DAISY recognition pin, and a unique Healer’s Touch sculpture designated for Nurse Leaders. In addition, the selected nurse’s department, clinic or facility will receive freshly baked cinnamon rolls for the nurse’s department on the day the award is presented.

How to Nominate an Extraordinary Nurse or Nurse Leader

Patients, visitors, family members, co-workers, and physicians may nominate a deserving CMH nurse or nurse leader by:

  1. Completing the electronic nomination form below.
  2. Filling out a nomination form found at the hospital/department/clinic/facility. Mail it to the address listed on the form or drop it into one of the DAISY Award submission boxes at CMH.
